How does semaglutide work? 

Semaglutide is a medication that binds and activates the GLP-1 receptor. GLP-1 is an important, gut-derived, glucose homeostasis regulator that is released after the oral ingestion of carbohydrates or fats. This cutting edge weight loss medication lowers blood sugar after meals by increasing insulin secretion and synthesis, GLP-1 suppresses Glc secretion, which slows gastric emptying, reduces food intake, Cravings and promotes beta-cell proliferation.

Many medical weight loss studies have shown success with the use of a weekly injection of semaglutide. Studies have shown that more than 85% of participants taking semaglutide lost 5% of their body weight, nearly 70% achieved a 10% weight loss, 50% achieved a 15% weight loss, and just under a third achieved a 20% weight loss.

How is it administered?

Patients will be sent home with a 2-4 week supply of semaglutide/Cyanocobalamin pre-drawn in a 100u insulin syringe and alcohol pads. They are a self-administered medication in the fatty abdominal area or thigh. Clean the area vigorously with alcohol for 30 seconds, Pinch fatty area of skin, insert needle in a 90 degree angle and slowly push plunger to administer medication. The sub-q injection should be administered once a week on the same day each week, any time of day, with or without food.

How long is the treatment plan?

The starting dose for semaglutide/Cyanocobalamin is  0.25 mg once weekly. After 4 weeks the dose may be increased to 0.5 mg once weekly. After at least 4 weeks with a dose of 0.5 mg once weekly, the dose can be increased incrementally up to 1 mg once weekly to further improve weight loss.

Dosage of semaglutide medication may be increased up to as high as 2.4 mg during the 16 to 20 weeks of the therapy with further clinical consultation with the medical director.

Failure to lose weight following appropriate guidelines may indicate an underlying hormonal or metabolic issue that may require further medical evaluation.

Possible side effects?

Most common side effects are GI related and include nausea, vomiting, constipation, heartburn  and bloating. There have been some reports of diarrhea. OTC supplements to consider for side effects are digestive enzymes, Pepcid, Tums, stool softener. For nausea recommendations to limit Nausea: Eat bland, low-fat foods, like crackers, toast and rice. Eat foods that contain water, like soup and gelatin. Avoid laying down after eating. Eat slowly. Go outdoors for fresh air. A prescription for anti nausea medication will be called into your pharmacy of choice.
